A lower body lift — also known as a belt lipectomy, circumferential abdominoplasty or 360 abdominoplasty — is the most comprehensive surgical procedure for removing excess skin and fat from the lower torso. Unlike a standard tummy tuck, which addresses the front of the abdomen only, a lower body lift operates circumferentiallytreating the abdomen, flanks, waist, outer thighs, lower back and buttocks in a single procedure through an incision that runs like a belt around the entire lower torso.


The procedure is most commonly performed after significant weight loss — whether through bariatric surgery (gastric bypass, sleeve gastrectomy, gastric banding), diet and exercise, or weight loss medication. After major weight loss, the skin often cannot retract to accommodate the body’s new shape, leaving loose, hanging folds of skin and fat around the entire midsection. A belt lipectomy removes this excess tissue definitively — no non-surgical treatment can achieve an equivalent result.

What Is a Lower Body Lift (Belt Lipectomy)?


A lower body lift is a circumferential body contouring proceduremeaning the incision extends all the way around the lower torso, treating the front, sides and back in one operation. The procedure combines the elements of an extended abdominoplasty (front and flanks) with a buttock lift (lower back and upper buttock) and an outer thigh lift, all through the same circumferential incision.


The incision runs just above the pubic hairline at the front, extending around the hips and continuing across the lower back at the level where the lower back and buttocks meet. This placement means the resulting scar can be concealed within the underwear and bikini line in most cases.


The procedure addresses several interconnected problems that develop after massive weight loss:


A lower body lift is distinct from a standard tummy tuck in scope, and distinct from an extended tummy tuck in that it also addresses the posterior — the lower back and buttocks. It is the most comprehensive single-session option for lower body skin excess.


The procedure goes by several names which are used interchangeably: lower body lift, belt lipectomy, circumferential abdominoplasty, circumferential lipectomy, and 360 abdominoplasty. All refer to the same circumferential procedure.

Choosing the right procedure depends on the distribution of skin laxity. Your surgeon will advise at consultation, but the following comparison explains the clinical distinctions.

Removes excess skin from the lower and mid-abdomen through a hip-to-hip horizontal incision. Includes muscle repair (diastasis recti plication) and umbilical repositioning. Does not address the flanks, lower back or buttocks. Appropriate where skin laxity is confined to the front of the abdomen.


Extends the horizontal incision around the hips to treat the upper outer flanks and waist. Appropriate where skin laxity spreads from the abdomen into the flanks. Does not address the lower back or buttocks.


Circumferential incision around the entire lower torso. Addresses the abdomen, flanks, waist, outer thighs, lower back and buttocks simultaneously. The most comprehensive option. Appropriate where skin laxity affects the entire lower body circumference — visible from front, sides and back. Longer operative time and recovery than the tummy tuck options above.


The key clinical indicator is posterior involvement — skin excess visible on the lower back and/or flat, descended buttocks. If a patient has significant skin laxity behind the hips as well as in front, a tummy tuck of any type will not address the posterior adequately. A lower body lift is the appropriate procedure.


Where the skin laxity is predominantly abdominal — with limited posterior involvement — an extended tummy tuck may be sufficient, and the surgeon will advise this at consultation. Where both horizontal and vertical abdominal skin excess is present, a may be more appropriate.

Preoperative marking
