Eight years ago I first using Belotero acid dermal fillers, made by Merz . I have not used acid filler since then.
The reason is straightforward: in eight years and several thousand treatments, I have not seen a single inflammatory .
That may not sound remarkable until you understand how common — and how accepted are with other . This post explains my journey, what eventually me to change, and what the scientific evidence tells us about why Belotero differently.
The Early Years of Hyaluronic Acid Fillers
Restylane was the world's first stabilised hyaluronic acid filler, launched in Europe in 1996. It represented a ground breaking advance — earlier that significant allergy risks, and results that were more and longer than the collagen-based that preceded them.
I began using in 2003. In a busy practice, inflammatory reactions were an occasional but fully expected part of the picture. Once or twice a year, a patient would develop a — from a small palpable nodule beneath the skin to a more generalised inflammatory requiring oral or hyaluronidase to dissolve the filler. These reactions could occur weeks or even months after treatment.
These were not considered a failing. They were a known risk and patients were routinely counselled about them as part of the consent .
Over the years I tried other hyaluronic acid as they came to market. The experience was broadly similar. The differed in consistency, longevity and handling characteristics, but the occasional reaction remained a constant. It was simply part of the landscape.
My views on dermal fillers are based on 20 years of clinical experience and the critical that comes from a and training.
A Prompt to Change
About eight years ago, I noticed an in the of reactions with a filler I was using at the time — Juvéderm, a widely-used and filler. It was enough to prompt me to reconsider my product choice, and I to the Belotero range of Acid (HA) filers.
Because had been so with previous products — once or twice a year at most — it took me three years to consciously register that I hadn't seen one since to Belotero. There was no moment of realisation. The was so gradual that I simply hadn't it.
But once I did notice, I began to pay close . Another year passed. Then another. Now, eight years and several thousand later, I have still not seen one single inflammatory reaction with Belotero.
Not one nodule. Not one generalised . Not one patient hyaluronidase or for a response
Why Does This Matter?
It is not a controlled trial. It is the of one practitioner, in one practice, over eight years. There are limits to what can be concluded from that alone.
But it is also not . Twenty years of comparative experience with multiple products, followed by eight consecutive years without a single reaction with one product, is clinically meaningful. It is the kind of pattern that — and, I believe, a change in .
My view is that many simply continue using other fillers remain and are so that their is never noticed. You do not miss what you have come to expect.
None of this is to that other hyaluronic acid fillers are unsafe. They are not. reactions with all established HA remain relatively uncommon, and many practitioners use other products to very good effect.
Equally, the that represent the most serious risks of any filler — which are related to injection technique and anatomy rather than the product itself — require the same regardless of which filler is used. choice does not replace clinical skill and anatomical knowledge.
What I am saying is this: in my clinical over twenty years, and by the now about particulate and tissue integration, behaves differently from other fillers in one specific and important respect. It does not cause reactions in my hands.
That is a significant clinical advantage — and one that I think is in the aesthetic community, because with other are so that their absence goes unnoticed.
What the Science Tells Us
Unlike most other acid fillers, which are cross-linked once during manufacture, Belotero is using a called Matrix (CPM) technology, in which the hyaluronic acid undergoes . This creates a gel with density — areas of higher and lower within the same — that gives it unique physical properties.
The result is that Belotero into tissue in a more homogeneous and way than other fillers. Histological comparing Belotero directly with Juvéderm and have shown that it distributes more evenly within the and architecture of the dermis, even the smallest spaces between fibres without disrupting the structural integrity of the tissue. Rather than sitting as a bolus, it becomes part of the tissue.
